I filled it in a few weeks back, didn't think to post a link here not sure why...
As a questionnaire it has a fine aim, but the leading questions will prevent it's analysis being taken too seriously. I agree with Chris, those were leading questions. I'm sure someone is going to use that fact against the results and they'd be right in doing so.
I'm also not happy with the alternatives they give.
